Please fill out the Student Contract and the List of Medications form and submit the forms to M-102 when you drop of your documentation and before you schedule an intake.
New Students should also print the OSD Disability Verification Form and submit it to their care provider and then return to the OSD office.
If you are a current student who has completed an intake you must fill out the following forms (if applicable) and return to OSD in person in order to receive these accommodations:
Students registered with the Office for Students with Disabilities(OSD) notify their professors of their accommodation needs by providing them with an Accommodation Letter signed by an OSD staff member.
The Accommodation Letter provides formal notification of students’ registration with OSD and their specific accommodation needs. In order to receive accommodations for their classes, students are required to present each of their professors with an Accommodation Letter. OSD strongly recommends that the student discuss the provisions of their accommodations with each of their professors upon presentation of the accommodation letter.
You must bring a copy of your schedule each semester to OSD in order to receive your letters. Please allow at least two business days for accommodation letters to be prepared by OSD.
